We have an open list and welcome requests for registration from patients living in or moving to the practice area. Please click here to see our boundary map.
You can register at Washingborough Family Practice by calling in and collecting a form and registration pack which are available from reception. You will be issued with a welcome pack containing a practice leaflet and various forms to complete to enable us to update our records.
Once we have received and processed your registration pack, and depending on the information you have provided, you may be invited to attend for a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
To save time you can also download and print a registration form. You will need to complete this by hand and bring it to the surgery. You can also complete an online application form.
PLEASE NOTE: THAT YOU ALSO NEED TO COMPLETE A NEW PT REGISTRATION PACK **WE WILL POST THIS OUT TO YOU AND ASK YOU RETURN IT TO US ONCE COVID-19 SOCIAL DISTANCING RULES HAVE ENDED**

New patient health checks
We offer all new patients a health check. If you wish to attend for a new patient health check, please let a member of staff know or call 01522 303600 to book an appointment.

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
